You can call your pharmacy to request a medication list if you do not have one. It is very important we have the most accurate list on the day you are seen. 

Insurance Card

I.d. card

A print out or physical insurance card will be accepted. If you have an HMO plan, please be sure Dr. Denysiak's name is listed on your card. Government-issued valid photo ID accepted. 

Medical records

If you have been seen out of state or if you have had a recent ER or Urgent Care visit, please bring a copy of your medical records for review. 

POLST Form or

advanced directive form

This is a document that lets us doctors know what your wishes are in the event something happens to you that affects your ability to make decisions for yourself. You can always change your mind verbally but it helps to have something in writing for yourself and your family members in the worst case scenario. We will provide you one in clinic if you need.

ABOUT US

Medicus M.D., Inc: a true family affair. Dr. Barbara Denysiak, M.D. has practiced as an Internal Medicine-trained generalist for over 30 years. Her daughter, Dr. Jacqueline Denysiak, M.D. is a Kaiser Permanente-trained, Board Certified  Family Medicine physician who joined the practice in 2020. The man behind the operation, Andrew Denysiak-- lovingly called "Andy"--functions as the practice manager. From their family to yours, The Drs. Denysiak bring comprehensive, broad-spectrum care to the San Diego community.

Dr. Denysiak's Longevity Book Club:
1.    "How Not To Die" by Dr. Michael Gregor MD - Examines the top causes of death in America. He explains how nutritional and lifestyle interventions can sometimes trump prescription pills and other pharmaceutical and surgical approaches, freeing us to live healthier lives.
2.    "Why We Sleep"  by Matthew Walker PhD -  Explains how we can harness sleep to improve learning, mood and energy levels, regulate hormones, prevent cancer, Alzheimer's and diabetes, slow the effects of aging, and increase longevity.

5.      "Outlive: The Science and Art of Longevity" by Peter Attia, MD, who wrote a manifesto on living a better and longer life that challenges the conventional medical thinking on aging and reveals new approach to preventing chronic disease and extending long-term health.

Yoga by the Ocean

Sick visits are to address a complaint, follow-up on previous visit, results review, administrative encounters, prescription refills, etc.

Well woman exams include: Pap smears, HPV testing when age-appropriate, pelvic exams, breast exams, STD testing, mammograms.

Removal of either Mirena/Skyla/Paraguard IUDs. These provide contraception for up to 5-7 years. 

Vaccinating A Child

Flu vaccines offered. 

Tdap vaccines offered.

Other vaccinations can be obtained at local Travel Clinics or pharmacies.

Nexplanon inserted into upper extremity after 2 negative pregnancy tests 2 weeks apart. Removal offered as needed.  Provides contraception for up to three years.

If you have a concerning lesion, a punch or shave biopsy can be obtained to determine the diagnosis of the lesion. 

Medical-Steroid-Injections

Knee and shoulder joint steroid injections are used to decrease inflammation in arthritic joints.

No Insurance? No problem.

Dr. Denysiak will see you for $250.00 per office visit. Please the fee schedule in the drop down menu above.

We offer face-to-face and telehealth appointments. 

Please make sure your insurance covers telehealth appointments before scheduling. 

You have the option to pay cash, check or credit card.

If you choose to pay with a credit card, please be aware there is an additional 10% transaction fee.

If you would like a refill on a medication, please call your pharmacy and have them send an electronic refill request to our office. (Please allow about  24 hours to process your request)
